All overhire employees are responsible for reporting their hours to the payroll department. The following form MUST be submitted with 24 hours of the shift worked to get paid. The form must be filled in completely. Any partially submitted forms will not be considered notification of shifts/hours worked. Please do NOT submit completed social security numbers via this form, only the last four digits. Our data base will find the correct employee based on the last four digits only. Any new employees must fill out the attached form and e-mail to payroll@advancedproductiongroup.com in addition to submitting the following form.
